Part One: General


1.1 Section Includes:

Plastic, single-use round forms for cast-in-place concrete applications, including the following:

  • Concrete piers and footings
  • Concrete light pole bases
  • Concrete pour-in-place bollards

1.4 Submittals

  • Submit under provisions of Section 01300
  • Manufacturer’s Data: Product data sheets and installation instructions


1.5 Delivery, Storage, and Handling

  • Deliver materials to site in manufacturer’s original, unopened containers and packaging with labels clearly identifying product name and manufacturer
  • Store forms in accordance with manufacturer’s instructions
  • Protect forms during handling and erection to prevent damage
  • Ceme-Tube forms must be stored upright prior to installation 
  • Ceme-Tube forms must not be allowed to roll around on their sides

2.1 Manufacturers

  • Requests for substitutions will be considered in accordance with provisions of Section 01600


2.2 Standard Concrete Pier Forms

Round Forms for piers and footings — Standard Ceme-Tubes by Ceme-Tube LLC with the following characteristics:

  • Material: Heavy-duty seamless HDPE plastic, impervious to moisture
  • Appearance: 8-, 10-, and 12-inch only. Uniform smooth appearance, without spiral form lines
  • Appearance: 18-, 24-, and 30-inch only. Integral molded-in reinforcement ribs for extreme backfill strength, and shape retention
  • Length: 4 feet (1219 mm)
  • Diameter: 8 inches (203 mm)
  • Diameter: 10 inches (254 mm)
  • Diameter: 12 inches (305 mm)
  • Diameter: 18 inches (457 mm); Gen 2 model with reinforcement ribs
  • Diameter: 24 inches (610 mm); Gen 2 model with reinforcement ribs
  • Diameter: 30 inches (762 mm); Gen 2 model with reinforcement ribs
  • Diameter: Refer to the drawings
  • Available optional domed cap: match size of forming tube
  • Integral collar for bracing and stacking on Standard Ceme-Tube form diameters up to 24 inches: integral collar on one end shall allow units to be braced and stacked to produce piers or footings up to 20 feet (six meters) tall in one pour
  • Integral collar for bracing and stacking on Standard Ceme-Tube form diameter of 30 inches: integral collar on one end shall allow units to be braced and stacked to produce piers or footings up to 12 feet (3.7 meters) tall in one pour


2.3 Light Pole Footing Forms

Round forms for light pole footings — Light Pole Ceme-Tube by Ceme-Tube LLC with the following characteristics:

  • Material: reflective yellow, architectural bronze, or concrete gray color above grade heavy-duty seamless HDPE plastic, impervious to moisture
  • Appearance: Integral molded-in reinforcement ribs for extreme backfill strength, and shape retention
  • Length: 4 feet (1219 mm)
  • Diameter: 18 inches (457 mm) with a 15-inch (381 mm) diameter hole on top of the tube to facilitate pouring, finishing, and light pole installation; Gen 2 model with integral reinforcement ribs (available in safety yellow only) 
  • Diameter: 24 inches (610 mm) with a 17-inch (432 mm) diameter hole on top of the tube to facilitate pouring, finishing, and light pole installation; Gen 2 model with integral reinforcement ribs (available in safety yellow, architectural bronze, or concrete gray) 
  • Diameter: 30 inches (762 mm) with a 17-inch (432 mm) diameter hole on top of the tube to facilitate pouring, finishing, and light pole installation; Gen 2 model with integral reinforcement ribs (available in safety yellow only) 
  • Integral collar for bracing and stacking on Standard Ceme-Tube form diameters up to 24 inches; integral collar on one end shall allow units to be braced and stacked onto Standard Ceme-Tube models to produce piers or footings up to 20 feet (six meters) tall in one pour 
  • Integral collar for bracing and stacking on Standard Ceme-Tube form diameter of 30 inches; integral collar on one end shall allow units to be braced and stacked onto Standard Ceme-Tube models to produce piers or footings up to 12 feet (3.7 meters) tall in one pour


2.4 Bollard Forms

Round forms for pour-in-place bollards — Bollard Assembly Ceme-Tube by Ceme-Tube LLC with the following characteristics:

  • Material: Safety yellow color above grade and standard black color below grade, heavy-duty seamless HDPE plastic, impervious to moisture
  • Appearance: Maintenance-free, uniform, and smooth
  • Length: 4 feet (1219 mm)
  • Diameter: 8 inches (203 mm)
  • Dome cap: included with hardware to match size of forming tube

Part Three: Execution


3.1 Examination

  • Examine areas to receive forming tubes
  • Notify architect if areas are not acceptable
  • Do not begin erection until unacceptable conditions have been corrected


3.2 Installation

  • Place and brace forming tubes in accordance with manufacturer’s instructions
  • Erect forming tubes at locations and to elevations as indicated on the drawings; erect forming tubes plumb
  • Do not use forming tubes that are damaged or contain defects that could impair exposed concrete surfaces
  • Provide protection on forms to prevent damage to exterior surfaces by adverse job site conditions before and after placement of concrete
  • Remove forms, if necessary, in accordance with manufacturer’s recommendations after concrete has achieved design strength
